It was "one of the most important actions against the production, distribution and possession of child pornography" in Spain, the national police said on Saturday.
A total of 125 homes and offices were searched in various parts of the country and around 500 terabytes of data "with extremely hard-core pornographic content" were confiscated, it said.
Of the 121 people detained, 118 were men and three women.
"More than 2 million files with serious sexual abuse of very young girls" were seized from one of the detained men in Madrid alone.
"Many of the videos were characterized by a high degree of violence towards the victims and were spread across 10 large hard drives," the statement said.
Investigations are continuing to identify further suspected perpetrators and victims.
